Information we collect
We collect information you provide through enrollment, attendance, quote, partnership, instructor-onboarding, and newsletter forms. This may include your name, email, phone number, employer, service preferences, dates, accessibility needs, typed acknowledgments, and documents you choose to upload. When a business purchases an inspection-readiness service, we may also collect establishment details, operational observations, temperature readings, corrective actions, manager acknowledgments, and photographs submitted as inspection evidence.
Approved contractor onboarding is handled through a private, identity-gated workflow. Signed contractor records are sent to a separate PostgreSQL onboarding database, encrypted before storage, and are not stored with ordinary website submissions. Tax-identification numbers are not requested through the website onboarding form.
How we use information
- To enroll participants and coordinate certification sessions
- To confirm attendance and provide requested support
- To review instructor, proctor, quote, and trade proposals
- To conduct private mock inspections, document findings, prepare reports, and track corrective actions
- To send transactional confirmations and requested updates
- To protect the website, prevent abuse, and maintain records
Payments
Payment-card information is collected in Square's secure embedded payment fields and processed by Square. Five Stars Food Safety receives a payment result and reference, but does not receive or store complete payment-card numbers in its website database.
Service providers
Limited information may be processed by hosting, database, file storage, email-delivery, spam-prevention, and payment providers that support the services requested. Information is not sold.
Website analytics
We use Google Analytics to understand aggregate website traffic, page usage, video engagement, and actions such as form completions or contact-link clicks. Analytics may use cookies and collect device, browser, approximate location, referral, and interaction information. We do not send form-entered names, email addresses, phone numbers, signatures, or uploaded documents to analytics. Google Signals and advertising personalization are disabled in our website tag.
Retention and security
Records are retained only as long as reasonably necessary for certification operations, legal obligations, dispute resolution, and legitimate business needs. Reasonable administrative and technical safeguards are used, but no online system can guarantee absolute security.
Your choices
You may ask to review, correct, or delete information associated with you, subject to recordkeeping and legal requirements. You may unsubscribe from promotional messages at any time.
